With the CHOC Links patient portal you can have access to your records anywhere there is an internet connection. This innovative approach is built on the idea that managing your healthcare information can be intuitive and efficient.
The CHOC Link patient portal is free of charge and can be accessed 24 hours a day, seven days a week.
With the portal you can:
- Send a message to the physician, advice nurse, the referral coordinator, the billing department, the office manager, or any member of our staff just by sending a message through a physician connection
- Receive reminders of upcoming appointments – via email notification or text message
- Receive results for most lab tests or procedures
- Receive office visit summaries
Minor proxy access is for children from birth through 12 years. This would be the child’s parent, step parent, grandparent, or others. Each proxy can have access to the child’s record with permission from the main proxy holder on the account on the record.
Young Adult access – Children 13 years old to 18 years old will not have access to the patient portal.
Adult access for those 18 and over allows full access to the health record. Those over 18 may request Adult Proxy access for a parent if they wish.
